Services

We provide services through the following care professionals in our team:

Skilled Nursing

Registered Nurses (RN)

  • Provides skilled services in accordance with the POC
  • Initiates the appropriate preventive and nursing procedures, processes orders and notifies MD of patient needs and changes in condition
  • Directs and supervises performance of CHHA’s and will report findings as appropriate
  • Participates in after hour on-call duty as assigned and coordinates services

Licensed Vocational Nurse (LVN)

Administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Therapy Services

Physical Therapy (PT)

Performs an evaluation/treatment to help patient to gain strength and coordination, and improve functional mobility, muscle tone, and endurance by teachings and body mechanics, with or w/o the assist of gait devices, exercise aids and various type of modalities

Speech Language Pathologist (SLP)

Involves treatments that help the patient with speaking, hearing and language problems. They perform evaluation, recommend restorative potential and provide therapeutic plan for voice disorders, dysphasia, language disorders, non-oral communication and articulation disorders.

Occupational Therapy (OT)

Evaluates and performs active and passive ROM, trains ADL’s/IADL’s and self/care, and recommends home modifications and functional mobility. They demonstrate and do muscle strengthening activities, and suggests and/or fabricate UE orthotics/splints.

Medical Social Services (MSW)

Provides assessment of social and emotional factors of patients needs for long term care. They promote community services and provide education to assist with strengthening the family. They counsel and assist with financial problem and entitlement, and help with the resolution of conflict related to illness.

Home Health Aide (HHA)

  • Assists professional staff by performing patient care duties at home, and performs household services essential to health care as directed by the Registered Nurse
  • Provides maintenance of clean, safe and healthy environment: hygiene, bathing, safe transfer techniques, nutrition; and
  • Performs simple procedures as an extension of the Skilled Nursing/Therapy services.
